WHAT YOU SHOULD BRING
Please bring a photo ID and an insurance card to your appointment. Failure to provide
this information will require payment for the full cost of the appointment
and any procedures performed at the time of service.
TRANSFERS FROM OTHER DOCTORS
If you are transferring your care from another physician to our practice, please have
your medical records faxed to our office prior to your visit. This will prevent
any confusion regarding previous treatments.

WILL INSURANCE COVER YOUR TREATMENT?
The Skin Firm accepts a variety of insurance plans for your convenience. Coverage for specific procedures varies by insurance carrier. We recommend that you contact your insurance company if you have any questions about the extent of your healthcare coverage and what your out-of-pocket costs might be. If you don’t have insurance coverage, we offer self-pay rates for you. We also accept cash, checks and major credit cards.
